A vitrectomy removes the vitreous — the clear gel filling the back of the eye — giving the surgeon direct access to the retina so problems at the back of the eye can be repaired. What the Operation Involves


Through tiny incisions in the wall of the eye, fine instruments remove the vitreous gel. The surgeon can then work directly on the retina — repairing tears, peeling membranes, or clearing blood. At the end, the eye is filled with saline, a gas bubble, or silicone oil, depending on what the retina needs in order to heal.


Conditions It Treats


  • Retinal detachment
  • Bleeding into the vitreous — often from diabetic eye disease
  • Macular holes
  • Epiretinal membranes that wrinkle the central retina
  • Severe eye infection or injury
  • Removing a lens fragment after complicated cataract surgery

Many of these simply cannot be treated any other way.


Will I Need to Posture?


Sometimes. If a gas bubble is placed, you may need to hold a specific head position for several days so it presses on the part of the retina needing support. It’s demanding, and it genuinely affects the outcome. Not every vitrectomy requires posturing — your surgeon will tell you clearly beforehand whether yours does.


Recovery


Vision is blurred at first and improves gradually. If gas was used, sight stays poor until the bubble absorbs over several weeks — you may see a wobbling line as it shrinks, which is normal. Most people return to light activity within a week or two, with vision improving over one to three months.

Do not fly or travel to high altitude while gas remains in the eye. The pressure change is dangerous.


Cataract Afterwards — Expected, Not a Complication


If you still have your natural lens, a cataract commonly develops in the months or couple of years after vitrectomy. This is expected, not a sign anything went wrong, and it’s straightforward to treat with routine cataract surgery when it becomes visually significant. Sometimes the two operations are combined — your surgeon will discuss this.
