Conditions We Treat
Trigger finger
The tendon that bends your finger catches as it passes through a tight sheath, so the finger clicks, snaps, or locks — typically worst first thing in the morning. Many settle with a steroid injection. Where they do not, a small day-care operation releases the sheath and cures it. It is commoner in people with diabetes.
Ganglion (a lump on the wrist)
Most wrist lumps are ganglions, and they are not cancer. They are harmless fluid-filled swellings from a joint or tendon sheath. Many need nothing done at all; some vanish by themselves. We remove them when they are painful, when they get in the way, or when the diagnosis is not certain.
A lump that is hard, growing, or painful at rest is a different matter and should be examined.
Cut tendons and nerves
A cut that leaves you unable to bend or straighten a finger, or that leaves a finger numb, has divided a tendon or a nerve. These need repair, and early repair gives a much better result. Do not wait for the wound to heal and then come.
Dupuytren’s contracture
A thickened cord in the palm slowly pulls the fingers down into the hand. It is painless, and it does not go away. Surgery is considered when you can no longer lay your hand flat on a table — the point at which waiting longer starts to cost you, because the finger joints themselves stiffen.
Hand and wrist fractures
Many are treated in a cast. Fractures that are displaced, rotated, or involve a joint surface need fixing — a rotated finger fracture that unites crooked will cross over its neighbour when you make a fist, and that is very difficult to correct later.
Hand Therapy Is Not Optional
This is the part patients skip, and it is the part that decides the result.
The hand stiffens extraordinarily quickly. A technically perfect tendon repair in a stiff hand is a failure. Hand therapy — splinting, controlled movement, and graded strengthening — is what converts a good operation into a working hand.
